About the role

  • Case Manager responsible for client-centered case management services and ensuring compliance with agency and state requirements. Handling client appointments, documentation, and assessments in a hybrid role.

Responsibilities

  • Schedule all client appointments monthly in Empathic as billable services.
  • Contact new clients within 1–2 business days to schedule appointments.
  • Document all rescheduling attempts and communication efforts.
  • Review client contacts and service delivery to ensure compliance.
  • Complete and maintain required assessments for each client.
  • Ensure all client files include necessary assessments and documents.
  • Document all client and collateral contacts within 3 business days.
  • Track and submit mileage and parking expenses within 30 days.
  • Carry a caseload of 12–15 clients.
  • Attend monthly Arubah trainings and other trainings assigned by the Treatment Director.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or a related field required.
  • Master’s degree in a behavioral health field preferred, especially for programs requiring diagnostic knowledge.
  • Minimum 1–2 years experience in case management, mental health services, social services, or community-based work.
  • Experience working with individuals with Serious Mental Illness (SMI) or high-needs populations strongly preferred.
