About the role

  • Interprets physician referrals and conducts speech and language evaluations for patients with various speech, language, voice, cognitive and swallowing disorders
  • Establishes and implements individualized plans of care and treatment based on evaluation results and functional goals
  • Monitors and evaluates outcomes, adjusts treatment plans, and provides status updates to physicians
  • Plans and completes patient discharge including written home programs, patient/family teaching, community referrals, and ordering equipment
  • Completes required documentation including progress notes, team conference reports, discharge summaries, statistics, billing, and scheduling
  • Applies insurance guidelines regarding authorization and number of visits
  • Cleans up after patient treatment, maintains equipment and area
  • Performs manual techniques, patient positioning, and transporting duties; uses modalities and exercise equipment per department orientation
  • Participates in performance improvement activities and manages assigned caseload
  • Pursues professional growth, orients new staff, and participates in co-worker evaluations as requested

Requirements

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
  • Speech-Language Pathology license issued by the state of Wisconsin
  • Master's Degree in Speech Pathology
  • No experience required
  • Intermediate computer skills
  • Excellent communication and interpersonal skills
  • Excellent organization, decision-making, and problem solving skills
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served; knowledge of growth and development over the life span
  • Team members will maintain annual educational requirements for hospital based certifications (e.g.DNV, CARF or others)
  • Physical requirements: sit, stand, walk, lift, squat; lift up to 50 lbs from floor to waist; lift up to 10 lbs from waist to overhead; lift and carry up to 40 lbs at waist height; push/pull with 30 lbs of force; perform a sliding transfer of 150 lbs with a second person present; repetitive use of hands; functional use of senses; ability to wear protective clothing; position may require travel
  • Operates all equipment necessary to perform the job
