About the role
- Social Work Services Program Consultant at the Department of Health conducting assessments for children's long-term care services. Collaborating with multidisciplinary teams and documenting outcomes within required timeframes.
Responsibilities
- Reviews CMAT referrals for appropriateness and to ensure complete referral information has been obtained to start the CMAT process
- Completes social work assessment interviews with necessary stakeholders to obtain information to complete psychosocial and developmental assessment
- Provides quality verbal and written communication with CMAT and community stakeholders to ensure understanding and engagement throughout the process
- Participates in community outreach, education, and engagement of community partners and stakeholders to promote awareness of CMAT program and referral process
- Documents all CMAT activities in the clients’ electronic health record
Requirements
- Must hold a valid, unrestricted Florida Driver's license and have access to personal transportation
- Must be able to work independently, prioritize and manage a daily schedule of activities
- Must have computer skills, including use of standard office machinery such as laptops, scanners, cell phones, printers and programs such as Outlook, PowerPoint, Excel and SharePoint
- An Associate's degree or higher in the social services preferred
- 2 years of medical setting experience and child welfare experience preferred.
