About the role
- Pharmacist Consultant evaluating and consulting on complex medication therapy for CareBridge Health. Working virtually with interdisciplinary teams and supporting member care plans.
Responsibilities
- supporting disease management program and physicians care plans through drug therapy evaluation and consultation
- evaluating complex member Rx needs and therapies through review of medication history
- assessing member compliance with plan through direct contact with members and physicians
- determining potential barriers to compliance and identifying solutions
- ensuring members understand appropriate use of current prescriptions
- reviewing medications with physicians and nurse practitioners
- serving as a resource to the clinical team in disease management and other clinical areas
- incorporating pharmacy information as part of managed care benefits
- researching new resources and providing education to team members
- consulting with multidisciplinary team regarding complex cases
- serving as departmental liaison to other areas or as representative on enterprise initiatives
- providing process and/or system guidance to lower level associates
Requirements
- BA/BS in Pharmacy
- minimum of 4 years of pharmacy and/or managed care experience
- current, valid, active and unrestrictive RPh license
- PharmD preferred
- medication therapy management experience
- experience working with electronic medical record systems
- experience with geriatric or medically complex patients preferred
- experience working virtually
- experience working with an interdisciplinary team
- proficiency using multiple computer systems
