About the role

  • Conduct thorough assessments of patients' physical conditions, mobility, and functional abilities to identify impairments and develop appropriate treatment plans.
  • Collaborate with patients, their families, and other healthcare professionals to establish individualized treatment goals and develop comprehensive plans that incorporate exercise programs, manual therapy, modalities, and other therapeutic interventions.
  • Provide hands-on physical therapy interventions, including therapeutic exercises, gait training, balance training, joint mobilizations, and other techniques, to help patients improve their strength, endurance, flexibility, and overall physical function.
  • Educate patients and their families on safe and effective therapeutic exercises, proper body mechanics, and strategies for managing pain, preventing further injury, and maintaining optimal physical health.
  • Maintain accurate and detailed documentation of patient assessments, treatment plans, progress notes, and outcomes in accordance with regulatory and reimbursement requirements.
  • Collaborate effectively with other healthcare professionals, such as physicians, nurses, occupational therapists, and speech therapists, to ensure coordinated care and maximize patient outcomes.
  • Communicate regularly with the patient's primary care physician and the home health team to provide updates on the patient's progress.
  • Adhere to all safety protocols, infection control measures, and legal and ethical standards to ensure the well-being and safety of patients during therapy sessions.

Requirements

  • Minimum 2 years experience in a home health
  • Bilingual a plus
  • Proficient with Kinnser/Wellsky EHR and OASIS procedures and charting process.
  • Current Texas state licensure or eligibility to practice as a Physical Therapist.
  • Bachelor's or Master's or DPT degree in Physical Therapy from an accredited institution.
  • Valid CPR BLS certification
  • Excellent interpersonal and communication skills to establish rapport with patients, families, and other healthcare professionals.
  • Ability to work independently, manage time efficiently, and prioritize tasks effectively.
  • Proficiency in documenting patient information accurately and maintaining electronic health records.
