About the role

  • Assist patients with education and assessment of their ongoing care management
  • Enhance quality of patient management and maximize patient satisfaction
  • Promote cost-effectiveness and increase efficacy of addressing healthcare disparities
  • Engage and care for the top 1% highest utilizing Medicaid patients
  • Assist patients with various financial and economic needs
  • Update team with important issues related to patient needs
  • Stay up to date with knowledge in this space

Requirements

  • Associates Degree (required)
  • Bachelor's Degree (preferred)
  • 1+ Years in outpatient Behavioral Health field and/or Social Support field
  • 2+ Years in direct care management position conducting biopsychosocial assessments and care plan development (preferred)
  • Understanding of how to navigate the Federal and State programs
  • Must possess a good understanding of disability, food stamps, and IHSS
  • Comfortable managing more than 80 patients
  • Bilingual (preferred)
  • Must live in Inland Empire and be available for in-person interactions as needed
