About the role
- Community Engagement Navigator assisting members in addressing social determinants of health barriers. Facilitating support and liaising between members and service providers.
Responsibilities
- Engage with members who have been identified as possessing significant SDOH barriers
- Work with the member to establish goals and next steps for addressing SDOH
- Support member in selecting provider or programs most appropriate for supporting their needs
- Act as a liaison between referral sources and the member
- Act urgently and appropriately for members expressing suicidal ideation or homicidal ideation and members having acute psychosis
- Perform multi-tasking including handling calls, texts, facsimiles, and electronic queues while simultaneously taking notes and speaking to customers
Requirements
- BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field
- 2 years experience with marketing, job development, job placement or social work
- any combination of education and experience which would provide an equivalent background
