About the role

  • Community Engagement Navigator assisting members in addressing social determinants of health barriers. Facilitating support and liaising between members and service providers.

Responsibilities

  • Engage with members who have been identified as possessing significant SDOH barriers
  • Work with the member to establish goals and next steps for addressing SDOH
  • Support member in selecting provider or programs most appropriate for supporting their needs
  • Act as a liaison between referral sources and the member
  • Act urgently and appropriately for members expressing suicidal ideation or homicidal ideation and members having acute psychosis
  • Perform multi-tasking including handling calls, texts, facsimiles, and electronic queues while simultaneously taking notes and speaking to customers

Requirements

  • BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field
  • 2 years experience with marketing, job development, job placement or social work
  • any combination of education and experience which would provide an equivalent background
