About the role

  • Provide proactive intervention and coordination of care to eligible Vaya Health members and recipients, ensuring appropriate assessment and services.
  • Perform comprehensive Health Risk Assessments (HRA) addressing bio-psycho-social factors and social determinants of health; administer PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS as needed.
  • Develop, support, and update person-centered Care Plans; link members with services across MH, SU, I/DD, TBI, physical health, pharmacy, LTSS and community resources.
  • Utilize Vaya’s Care Management software/AHR to document assessments, medication lists, care plans, and monitor follow-through; ensure compliance with federal/state/Vaya requirements.
  • Participate in interdisciplinary care team meetings, transitional care management, diversion efforts, and high-risk team meetings; collaborate with RNs, pharmacists, MDs, providers, and community stakeholders.
  • Provide clinical consultation to providers, review clinical assessments for accuracy, and offer recommendations for appropriate care and placement decisions.
  • Conduct outreach, engagement, crisis intervention, and coordinate community-based supports; facilitate transitions from hospitals and residential facilities and track discharges.
  • Monitor services for quality, verify Medicaid eligibility, escalate safety concerns, participate in trainings and Vaya committees, and perform other duties as assigned.

Requirements

  • Master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
  • For incumbents with a Master’s Degree in a Human Services Area besides Nursing, two (2) years of experience working directly with individuals with BH conditions or two (2) years of prior Long-tern Services and Supports and/or Home Community Based Services coordination, care delivery monitoring and care management experience.
  • For incumbents with a Master’s Degree in Nursing, four years of full-time accumulated experience in mental health with the population served is required.
  • Must meet the criteria of being a North Carolina Qualified Professional with the population served in 10A NCAC 27G .0104.
  • Licensure/Certification Required: Valid licensure required. Acceptable licenses are Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Clinical Social Worker Associate (LCSWA), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Clinical Mental Health Counselor Associate (LCMHCA), Licensed Clinical Mental Health Counselor Supervisor (LCMHCS), Licensed Psychological Associate (LPA), Health Services Professional Psychological Associate (HSP-PA), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Addiction Specialist Associate (LCASA), Licensed Marriage and Family Therapist (LMFT) or Licensed Marriage Family Therapist Associate (LMFTA).
  • Ability to express ideas clearly/concisely and communicate in a highly effective manner.
  • Ability to drive and sit for extended periods of time (including in rural areas).
  • Proficiency with Vaya’s Care Management software platform/administrative health record (AHR) and intermediate to advanced proficiency in Microsoft office products (Word, Excel, Power Point, Outlook, Teams, etc.).
  • Knowledge of HIPAA requirements, Authorization for Release of Information (ROI) practices, and documentation compliance.
  • Ability to administer and interpret screenings (PHQ-9, GAD, CRAFT, ACES, LOCUS/CALOCUS) and perform comprehensive Health Risk Assessments (HRA).
  • Experience and knowledge of the NC Medicaid program, NC Medicaid Transformation, Tailored Plans, state-funded services, and accreditation requirements are preferred.
  • Ability to complete and maintain all trainings and proficiencies required by Vaya.
