Responsibilities
Work focusing on our Trinity Health medical group patients.
Focus on patients with multiple chronic diseases, CHF, COPD.
Focus on disease education, identifying and mitigating social influencers of health.
Coordinate needed services and collaborate with the provider offices to ensure integrated care through the healthcare continuum and improved health outcomes of our patients.
Requirements
Associates Degree required, BSN preferred.
Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting.
Certification in Case Management (CCM) preferred.
5 years of RN experience.
Prior electronic medical record (EMR) experience required.
Previous project leadership experience required.
Possess strong community resource knowledge.
General knowledge of insurance plans and clinical resource management.
Excellent oral and written communication skills.
Excellent customer service skills.
Ability to work independently, setting priorities to coordinate care plan efficiently.
Proven leadership skills and ability to work effectively in a team environment.
Highly organized and detail-oriented, ability to perform multiple tasks.
Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support.
Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences.
