About the role

  • RN Care Manager focusing on chronic disease management for Trinity Health patients. Collaborating with provider offices to improve health outcomes through integrated care.

Responsibilities

  • Work focusing on our Trinity Health medical group patients.
  • Focus on patients with multiple chronic diseases, CHF, COPD.
  • Focus on disease education, identifying and mitigating social influencers of health.
  • Coordinate needed services and collaborate with the provider offices to ensure integrated care through the healthcare continuum and improved health outcomes of our patients.

Requirements

  • Associates Degree required, BSN preferred.
  • Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting.
  • Certification in Case Management (CCM) preferred.
  • 5 years of RN experience.
  • Prior electronic medical record (EMR) experience required.
  • Previous project leadership experience required.
  • Possess strong community resource knowledge.
  • General knowledge of insurance plans and clinical resource management.
  • Excellent oral and written communication skills.
  • Excellent customer service skills.
  • Ability to work independently, setting priorities to coordinate care plan efficiently.
  • Proven leadership skills and ability to work effectively in a team environment.
  • Highly organized and detail-oriented, ability to perform multiple tasks.
  • Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support.
  • Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences.
  • Incorporate strategies for interacting with persons from diverse backgrounds (e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, and affiliation, mental and physical capabilities).
