Intensive Case Manager/Housing Worker providing case management support to asylum seekers and refugee claimants. Collaborating with community partners to ensure successful housing stability and transition.
Responsibilities
Provide intensive, individualized case management support to asylum seekers and refugee claimants residing in Wesley-operated transitional housing, with the goal of stabilization, independence, and transition to permanent housing.
Conduct comprehensive intake and needs assessments for clients being referred into the transitional housing program, including coordination with Halton Region’s shelter intake system and/or Peel Region’s Reception Centre to ensure smooth and timely transfers.
Deliver mobile and on-site case management services addressing housing stability, income supports, health and mental health needs, settlement services, and other resettlement priorities.
Connect clients to essential community, regional, provincial, and federal services, including Ontario Works, healthcare providers, Legal Aid, mental health supports, food security programs, IRCC-related services, and other settlement resources.
Support clients in navigating immigration-related processes, including booking and attending required IRCC appointments such as the Refugee Medical Exam (RME), and providing orientation to Canada’s settlement and service systems.
Provide intensive housing assistance, including housing planning, landlord engagement, referrals, and support with applications, to help clients secure and transition into permanent housing in their community of choice.
Collaborate closely with the Program Housing Worker and Wesley’s Housing & Homelessness team to ensure coordinated admissions, client flow, and continuity of care within the transitional housing program.
Coordinate interpreter and cultural supports as required, ensuring services are trauma-informed, client-centered, and responsive to language, age, health, and cultural needs.
Maintain accurate case notes, reports, and data related to client progress, outcomes, and challenges, contributing to program reporting and continuous improvement.
Work collaboratively with Wesley’s Newcomers programs to integrate services, apply evidence-based practices, and identify efficiencies and system improvements.
Participate in flexible and rotating schedules as required to meet program needs, including support for occasional extended or after-hours intake coordination.
Attend regular supervision, team meetings and attend mandatory organizational training and professional development.
Requirements
Post-secondary education at a College Diploma level or higher, including studies pertinent to settlement, social services, adult education, community health, and migration issues.
At least two years of relevant experience in case management, residential settings, or working with vulnerable populations (e.g., individuals experiencing homelessness, Newcomers, mental health issues).
An understanding of specific frameworks like the Housing First model, harm reduction practices, and anti-oppressive practices is an asset/valued.
Advanced computer skills required in the following programs: Microsoft Word, PowerPoint, and Outlook; Adobe; any program database.
Excellent communication, organization, and coordination skills.
Proven ability to demonstrate Wesley’s mission, vision, and values.
Fluency in Arabic, French, or Swahili, (verbal and written) in addition to English is required.
Available to work flexible/irregular hours, including evenings and weekends.
A Valid Driver’s License with access to a vehicle would be an asset.
Non-violent crisis intervention and 1st aid/CPR training is required.
