About the role

  • LTSS Service Coordinator facilitating service coordination for individuals in specialized programs. Conducting assessments and managing physical and behavioral health needs in Florida communities.

Responsibilities

  • Conducting service coordination functions for a defined caseload of individuals in specialized programs
  • Facilitates the Person Centered Planning process
  • Conducts assessments and develops comprehensive Person Centered Support Plans
  • Manages individuals’ physical health, behavioral health, and LTSS needs
  • Documents service and support goals in collaboration with care teams
  • Submits utilization/authorization requests to management
  • Responsible for reporting critical incidents and assisting in appeals or grievances

Requirements

  • Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency
  • Bilingual in English/Spanish strongly preferred
  • LTSS, case management, social work or hospital discharge planning experience preferred
