Adult Case Manager providing comprehensive case management services for families at Mountain Home. Advocating and connecting families to essential medical, social, educational, vocational, and recreational services.
Responsibilities
Providing direct support to clients, including assistance with paperwork, goal planning, and skill-building.
Maintaining accurate and timely client documentation, including dates, times, and progress notes for all meetings, in the electronic health record system.
Building rapport with clients while maintaining professional boundaries at all times
Providing services primarily in the community or clients’ homes
Providing staff with client history/special requirements and hosting team meetings
Making referrals to other services such as mental health services, parenting, etc.
Communicating with clients’ therapists for care coordination and treatment planning
Providing transportation and serving as an advocate for clients at appointments
Completing and overseeing the discharge process for all assigned clients
Meeting and maintaining billing hours expectations and documentation timelines
Attending meetings and training as required and meeting regularly with their supervisor
Other appropriate and job-related tasks as assigned
Requirements
Bachelor’s degree in social work, psychology, rehabilitation counseling, education, vocational rehabilitation, or a similar field and/or related experience
At least 2-3 years of experience in supported employment, vocational rehabilitation, mental health services, or related fields.
Experience working with individuals with mental health conditions, disabilities, or other barriers to employment is strongly preferred.
Familiarity with the Individual Placement and Support (IPS) model or similar evidence-based employment programs is preferred.
Certification in Supported Employment (CESP) or related credentials are a plus
CPR/First Aid certification (or willingness to obtain it within a designated time frame) is preferred.
Any state-specific licensure or certification (e.g., Licensed Professional Counselor, Licensed Social Worker) is preferred but not required, depending on program needs.
Familiar with social services in the Missoula community
Experience providing services to young adults with special needs, severe emotional disturbances, and/or cognitive or developmental delays.
