About the role

  • Care Manager (RN) responsible for care plan oversight and management for high-risk members at Maryland Care Management. Leading interdisciplinary team collaboration and implementing care strategies for special needs individuals.

Responsibilities

  • Responsible for planning, development, implementation, and oversight of care plans
  • Manages the care management process for high-risk and special needs members
  • Conducts assessments and collaborates with physicians to determine medical needs
  • Initiates, develops, plans, and implements care plans for members
  • Maintains care management records and ensures data integrity in health plan systems
  • Educates medical providers about members' special needs
  • Coordinates with community support and social service systems

Requirements

  • Registered Nurse with current state license
  • 2+ years of clinical nursing experience in acute care or community setting
  • 2 years of care management experience
  • Demonstrated experience helping members using community agencies
  • Knowledge of Managed Care Programs (preferred)
  • BSN (preferred)
  • CCM (preferred)
