About the role

- Provides mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members
- Coordinates care for patients and assists them in accessing services

- Provides case management and care coordination services for clients/patients
- Acts as a liaison between patients, referral sources, and funders

- Participates in quality improvement activities for the department or program
Requirements
- Master’s Degree Social Work, Counseling, or related field
- Ability to remain patient and calm in stressful situations
- Knowledge of community resources and referrals to community and governmental agencies for services
- Understanding of the psychosocial/medical needs of clients for treatment plans
