About the role

Responsibilities
- Manage requests for support and exits from La Roche’s services for the people concerned.
- Conduct the initial assessment of needs and co‑construct, with the person seeking support, their individualized pathway plan.
- Develop and maintain partnership relationships as part of the person’s pathway.
- Provide strengthened coordination according to needs, particularly during transitions between services and/or following significant events in the person’s daily life.
Requirements
- Knowledge of Law 2002-2 and Law 2005 (French social/health legislation)
- Knowledge of the reports and documents associated with/derived from the “0 sans solution” report
- Knowledge of recovery and psychosocial rehabilitation
- Experience with network-based work and partnership approaches
- Knowledge of the local institutional and associative landscape
- Knowledge of the association’s mission and La Roche’s unit projects
