Responsibilities
Manage requests for support and exits from La Roche’s services for the people concerned.
Conduct the initial assessment of needs and co‑construct, with the person seeking support, their individualized pathway plan.
Develop and maintain partnership relationships as part of the person’s pathway.
Provide strengthened coordination according to needs, particularly during transitions between services and/or following significant events in the person’s daily life.
Requirements
Knowledge of Law 2002-2 and Law 2005 (French social/health legislation)
Knowledge of the reports and documents associated with/derived from the “0 sans solution” report
Knowledge of recovery and psychosocial rehabilitation
Experience with network-based work and partnership approaches
Knowledge of the local institutional and associative landscape
Knowledge of the association’s mission and La Roche’s unit projects
