About the role

- Planning and coordination of admission and discharge management services as well as post-discharge patient care.

- Ensuring interdisciplinary and cross-organizational cooperation.
- Managing discharge processes in close cooperation with the social services team.
- Maintaining close contact with patients and their relatives and intervening in case management when needed.
Requirements
- Successfully completed training as a nurse (health and nursing) or as a medical assistant.
- High level of professional competence.
- Additional qualification in Case Management (DGCC) or willingness to obtain it.
- Open and positive attitude towards the intended area of work as well as commitment, assertiveness and flexibility.
- High resilience and reliability.
- Empathy and helpfulness.
