Senior Director managing development and implementation of integrated care management programs at IEHP. Focusing on person-centered care that addresses medical, behavioral, and social needs historically underserved populations.
Responsibilities
Plan, develop and implement an integrated care management program that focuses on a person-centered model of care which takes in to account the member’s medical, behavioral, and social needs.
Partner in the development of interventions in the communities to address the social determinants of health that impact the quality of our members lives.
Ensure all private and government program regulations and requirements are met.
Commitment to incorporate IEHP’s Quality Program goals including, but not limited to, HEDIS, CAHPS, and NCQA Accreditation.
Requirements
Minimum of ten (10) years progressively responsible experience in managed care setting that serves the Medicaid population and/or similar populations.
Seven (7) or more years of progressive healthcare leadership.
Strong clinical experience in multidisciplinary team collaboration.
Proven leadership experience, mentoring, and developing a team at a leadership level.
Required: Bachelor's degree in a related field from an accredited institution required.
Possession of an active, unrestricted, and unencumbered license in one of the following areas required : Registered Nurse (RN) license issued by the California BRN Licensed Clinical Social Work or Licensed Marriage Family Therapy issued by the California Board of Behavioral Sciences
