About the role

  • Provide case management and support to adults in the community.
  • Conduct home and community visits to complete nursing assessments.
  • Establish and monitor wellness and health promotion activities.
  • Re-evaluate the member’s health and wellness at appropriate intervals.
  • Perform routine health monitoring activities based on member needs.
  • Build constructive working relationships with members and families.
  • Assess and implement interventions in crisis situations.
  • Evaluate members’ ability to safely manage their medication and medical care.
  • Ensure appropriate services and supports are provided to meet member needs.
  • Assist in developing program policies and procedures.

Requirements

  • Must be a Registered Nurse, licensed in the State of Wisconsin.
  • Two (2) years of related health care experience.
  • Must be able to travel to any location within Dane, Adams, Columbia, Dodge, Green Lake, Iowa, Jefferson, Marquette, Rock and Waushara counties.
  • Proven ability to learn new software applications.
  • Bachelor’s degree in nursing (BSN) preferred.
  • Previous home health, managed care or case management experience preferred.
  • Previous experience working with persons with disabilities and/or older adults in a health care setting preferred.
  • Case Manager Certification preferred.
  • General knowledge of Medicaid and Medicare benefits preferred.
