About the role

  • Social Work Case Manager supporting patients in managing chronic illnesses through effective psychosocial interventions and resources. Working virtually to promote productive healthcare outcomes.

Responsibilities

  • Responsible for ensuring effective psychosocial intervention
  • Manages behavioral and psychosocial needs
  • Assists members to effectively utilize available resources
  • Evaluates members' ability to independently manage self
  • Provides guidance to members seeking alternative solutions
  • Evaluates members' strengths related to health self-management
  • Facilitates and coordinates behavioral health resources as individual member needs are identified

Requirements

  • Requires MS (at a minimum) in Social Work
  • minimum of 3 years of experience in case management in a health care environment
  • Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s) required
  • Bilingual (Spanish) or multi-language skills preferred
  • Experience with social determinants/drivers of health is strongly preferred
  • Experience working in a medical office (dialysis, oncology, etc.) is preferred
