About the role
- Behavioral Health Case Manager managing cases for members with behavioral health issues and substance use disorders. Conducting assessments, developing care plans, and coordinating resources for member care.
Responsibilities
- work with member population with serious emotional disturbance and/or substance use disorder
- meet with members in person as per contractual requirements
- conduct assessments to identify individual needs and develops care plan
- to address objectives and goals as identified during assessment
- monitor and evaluate effectiveness of care plan and modifies plan as needed
- support member access to appropriate quality and cost-effective care
- coordinate with internal and external resources to meet identified needs of the members
Requirements
- MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing
- minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
- Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW or Clinical Psychologist
- Preferred qualifications: experience in case management and telephonic coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders
