About the role

  • Behavioral Case Manager II providing case management services to individuals with serious mental illness. Requires a master's degree and clinical experience for Virginia residents.

Responsibilities

  • Provide case management services to individuals with serious mental illness (SMI)
  • Conduct regular outreach to assigned members to build rapport and assess needs
  • Link members to needed services/resources and provide short-term support (up to 6 months)
  • Respond to complex cases and account specific requests
  • Conduct assessments to identify individual needs and develop care plans
  • Monitor and evaluate effectiveness of care plans and modify as needed
  • Coordinate with internal and external resources to meet identified needs

Requirements

  • Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing
  • Minimum of 3 years of clinical experience in social work counseling
  • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist
  • Previous experience in case management and telephonic and/or in person coaching
  • Experience with complex psychiatric and substance abuse disorders
  • Knowledge of behavioral health systems of care in Virginia strongly preferred.
