About the role

  • Clinical Consultant Pharmacist at CareRx providing pharmacy services to optimize resident outcomes. Collaborating with clinical teams while ensuring regulatory compliance and personalized care.

Responsibilities

  • Act as a liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support
  • Travel to Special Care Homes and Personal Care Homes
  • Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships
  • Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy
  • Complete all required documentation and billing processes as required through SPDP, the Saskatchewan College of Pharmacy Professionals regulations, and other standards and regulations
  • Provide education to Residents and staff on various health-related topics
  • Support CareRx Pharmacy policies and procedures within retirement communities and long-term care homes
  • Ensure regular on-site visits are made to all assigned clients
  • Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
  • Be available for special clinical team projects as required
  • Works collaboratively with other team members to support applicable homes and communities
  • Help with pharmacy operations/drug distribution when required
  • Attend and actively participate in clinical team meetings
  • Coordinate and actively participate in seasonal vaccination activities (fall and spring)

Requirements

  • Insured and currently licensed in good standing with the Saskatchewan College of Pharmacy Professionals
  • Proven ability to foster strong partnerships with other members of the healthcare team
  • Effective organization, planning, and time management skills
  • Knowledge of Kroll Windows application and Microsoft Office programs
  • Must be familiar with the provincial standards of practice established for SCH and PCH
  • Excellent customer service and interpersonal skills, oral and written communication skills and be able to interact with persons of varying levels of professional expertise
  • Excellent time management skills
  • Skilled in critical thought processes and be able to prioritize appropriately
  • Must demonstrate initiative and creativity
  • Self-directed requiring minimal supervision and able to work independently
  • Motivated to achieve results that reflect a high standard of quality
  • Good listening skills and be adaptable to changes in job environment
  • Dependable and responsible in the execution of job duties
  • Adaptable to changes within internal work structures
  • Strong presentation and teaching skills
  • Must possess a valid driver’s license
