About the role

  • Continuing Care Manager overseeing the continuum of care across outpatient services and sober living residences. Coordinating treatment journey and supporting clinical, operational, and administrative needs.

Responsibilities

  • Coordinate all step-downs and transitions between levels of care, ensuring continuity and completion of case management needs
  • Support clinical interventions aimed at increasing client engagement, improving attitudes and behaviors, and reinforcing recovery-oriented goals
  • Collaborate with clinical leadership on implementation and facilitation of 12-step or other clinical curricula
  • Attend weekly staffing meetings (Mondays) to review client progress, needs, and potential intervention
  • Conduct environment-of-care walkthroughs at sober living residences 3–4 times per week
  • Directly supervise House Managers across sober living and transitional living homes
  • Oversee the sober living intake process and support admissions, transitions, and discharges
  • Serve as on-call support for sober living crises, including relapse events, behavioral issues, outbursts, or urgent home repairs
  • Implement behavioral agreements, administrative discharges, and therapeutic transfers as needed
  • Maintain and update the LRR Census Spreadsheet
  • Maintain and update the LRR Reconciliation Sheet
  • Ensure LRR EMR platform records are accurate and up to date for all intakes and discharges
  • Support the Director of Outpatient Services by maintaining accurate and timely documentation within the EMR platform
  • Ensure Austin Outpatient EMR platform records are kept current and accurate
  • Manage supply ordering for sober living residences
  • Oversee rent collection processes for sober living houses in coordination with the Business Office
  • Provide transportation for PHP clients to programming on Mondays and Thursdays
  • Support PHP intakes at the outpatient facility on Thursdays
  • Assist outpatient and sober living teams with day-to-day administrative and operational needs

Requirements

  • High level of organization, follow-through, and attention to detail
  • Strong communication and interpersonal skills
  • Ability to manage multiple priorities across clinical and operational environments
  • Comfort responding to crisis situations in sober living environments
  • Proficiency with electronic health record systems (EMR platforms) or willingness to learn
  • Clinical licensure or active pursuit of clinical licensure strongly preferred
  • Prior experience in behavioral health, substance use treatment, outpatient services, or sober living operations
