Responsibilities

  • Evaluates the patient's comprehensive health care needs in the outpatient clinic and implements an appropriate plan according to those needs utilizing the physician, community resources and the patient's support system.
  • Utilize critical thinking skills for the care of the patient.
  • Provides skilled therapy procedures/assessments and instructs patients and their families in the plan of care.
  • Utilize electronic records to document treatment & care.

Requirements

  • Current active or current temporary Kentucky PT or OT License
  • Must have CHT
  • BLS certification within 90 days of hire
  • Ability to work 40 hours per week with occasional late or early shifts to accommodate patients
