Responsibilities
- Evaluates the patient's comprehensive health care needs in the outpatient clinic and implements an appropriate plan according to those needs utilizing the physician, community resources and the patient's support system.
- Utilize critical thinking skills for the care of the patient.
- Provides skilled therapy procedures/assessments and instructs patients and their families in the plan of care.
- Utilize electronic records to document treatment & care.
Requirements
- Current active or current temporary Kentucky PT or OT License
- Must have CHT
- BLS certification within 90 days of hire
- Ability to work 40 hours per week with occasional late or early shifts to accommodate patients
