About the role

  • Service Coordinator scheduling and supervising in-home care workers and clients in Albuquerque, NM. Conducting quality assurance through home visits and client monitoring.

Responsibilities

  • Responsible for scheduling and supervising in-home care workers and clients in a geographic area.
  • Conducting in-home visits at clients' homes to monitor and report on quality of services being provided by Home Care Aides.
  • Determine whether existing care plans are adequate to meet client needs.
  • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client requests, illness, vacation or leaves of absence.
  • Provides alternate coverage to ensure the clients care plan is followed and client services are not interrupted.
  • Contacts care providers and clients to provide service updates.
  • Monitors clients condition and performance of duties by Home Care Aides.
  • Maintains all company and Department on Aging rules, regulations, and standards.
  • Conducts home visits to new clients to welcome and review the Welcome Packet.
  • Observes and evaluates Home Care Aides in the client's home as directed by Supervisor.
  • Prepares and completes accurate evaluation reports and case notes as needed.
  • Maintains schedules to assure timely completion of all assigned home visits.
  • Attends weekly branch meetings.
  • Maintains a high degree of confidentiality at all times due to access to sensitive information.
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department.
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures and standards.
  • Performs other duties as assigned.

Requirements

  • Must have high school diploma or equivalent.
  • 1 year of Industry experience required
  • Interpersonal, organizational and communication skills.
  • Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
  • Must have reliable transportation.
