Working with community resources to build unique resources for clientele to support isolation and community accessibility.
Experience working with adults with mental health and addictions issues and people experiencing homelessness and providing support to them.
Experience working with older adults who are dealing with social isolation.
A passion for supporting communities in need through client-centered housing approaches that focus on the social determinants of health.
Knowledge of the recovery and harm reduction models.
Ability to deal with confidential information and possess excellent interpersonal and diplomacy skills.
Demonstrated ability to form helping relationships based on trust and empowerment.
Evidence as a highly motivated team player, with proven ability to take initiative and be self-directed.
Must be able to meet travel requirements of the job and be available to work on site in various locations across Halton (Burlington, Oakville, Milton, Georgetown and Acton) as well as traveling within the Halton community.
Must be able to work flexible shifts depending on client needs. Connecting with clients in their homes, in the community or other areas within Halton Region.
Work flexible shifts, including evenings and weekends, to meet the demands of a year-round program schedules and needs.
Requirements
Post-secondary degree/diploma in Human Services or related field with a background in recreation leadership and/or related experience.
Experience in case management techniques, non-violent crisis intervention, and First Aid/CPR.
Knowledge of and sensitivity to the life experiences and diverse needs, identities, and cultural backgrounds of individuals in underserved communities and skilled in establishing trusting relationships.
Knowledge of the community, including local programs and services.
Demonstrated ability to work independently, as part of a team, and across multiple sectors.
Computer and social media literacy.
