About the role

  • Lead and manage care managers delivering CFSP care management for children and families. Ensure clinical quality, trauma-informed care, and community coordination at Elevance Health.

Responsibilities

  • Manage and oversee assigned care managers and ensure fidelity to the CFSP Care Management model which includes physical health, behavioral health, and social services
  • Manage resource utilization to ensure appropriate delivery of care to members and adequate coverage for all tasks and job responsibilities
  • Review all Care Plans and ISPs for quality control and provide guidance to care managers on addressing Members’ complex health and social needs
  • Ensure care managers provide Trauma-Informed Care and recognize the impact of ACEs on the CFSP population
  • Coordinate service delivery to include member assessment of physical and psychological factors
  • Participate in cross-functional workgroups to maintain and develop program
  • Evaluate current processes of Special Program's support functions and recommend changes for increased efficiencies and improved outcomes
  • Develop and conduct training programs for staff involved in the program
  • Extract and manipulate analytical data to present findings to relevant markets and stakeholders
  • Hire, train, coach, counsel, and evaluate performance of direct reports
  • Field-based role: travel to client sites or designated locations and occasional office attendance for meetings or training

Requirements

  • North Carolina residency is required
  • You should live within Franklin/Warren/Halifax Counties in Region 5
  • Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina
  • Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing
  • Requires three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system
  • At least 2 years of management/supervisor experience (with direct reports) is needed for this position
  • We are unable to accommodate LCSW-A, LCMHC-A or any other associate level licenses
  • Travel within your assigned area is required (field-based role)
  • When not in the field, work virtually from home
