About the role

  • Provide comprehensive and compassionate nursing care to patients in their homes
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and their families
  • Conduct thorough assessments of patients' physical and psychological status
  • Administer medications according to physician orders
  • Provide education and support to patients and their families
  • Offer emotional support and counseling to patients and families
  • Collaborate with the interdisciplinary team to ensure continuity of care
  • Maintain accurate and up-to-date patient records
  • Provide bereavement support to families and caregivers
  • Stay informed about current trends and advancements in hospice and palliative care

Requirements

  • Current and unrestricted RN license in the State of Texas
  • Minimum of 2 years of proven clinical experience in hospice, palliative care
  • Bilingual English/Spanish preferred
  • Proficient with computers: Axxess EHR or other hospice EHR; email; able to use Slack and other platforms
  • Strong case management skills, including assessment, care planning, coordination, and documentation
  • Ability to provide compassionate and empathetic care
  • Excellent verbal and written communication skills
  • Demonstrated ability to work reliably and collaboratively in an interdisciplinary team
  • Familiarity with hospice regulations, including Medicare Conditions of Participation and Joint Commission standards
  • Reliable transportation
