About the role
- Registered Nurse providing compassionate hospice care for patients in their homes with a focus on emotional support and individual care plans. Collaborating with interdisciplinary teams for quality end-of-life care.
Responsibilities
- Provide comprehensive and compassionate nursing care to patients in their homes
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and their families
- Conduct thorough assessments of patients' physical and psychological status
- Administer medications according to physician orders
- Provide education and support to patients and their families
- Offer emotional support and counseling to patients and families
- Collaborate with the interdisciplinary team to ensure continuity of care
- Maintain accurate and up-to-date patient records
- Provide bereavement support to families and caregivers
- Stay informed about current trends and advancements in hospice and palliative care
Requirements
- Current and unrestricted RN license in the State of Texas
- Minimum of 2 years of proven clinical experience in hospice, palliative care
- Bilingual English/Spanish preferred
- Proficient with computers: Axxess EHR or other hospice EHR; email; able to use Slack and other platforms
- Strong case management skills, including assessment, care planning, coordination, and documentation
- Ability to provide compassionate and empathetic care
- Excellent verbal and written communication skills
- Demonstrated ability to work reliably and collaboratively in an interdisciplinary team
- Familiarity with hospice regulations, including Medicare Conditions of Participation and Joint Commission standards
