Hybrid Clinical Supervisor – Physical Therapy, Home Health

About the role
- Ensure that patient care is coordinated and managed appropriately
- Serve as the first point of contact for field clinicians to ensure timely support
- Supervise clinical personnel and provide in-field oversight of clinicians providing home-based care
- Conduct supervisory visits of field clinicians (approximately 12 patient visits per week)
- Coordinate training and educational programs for field clinicians and assess staff learning needs
- Perform administrative call duties and serve as a role model and resource for staff
- Coordinate bi-weekly case conferences to review patient progress and revise care plans
- Review and evaluate each case, including clinical diagnosis, medications, procedures and clinical course
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care
- Be available during operating hours to assist clinicians and occasionally provide coverage outside primary territory as needed
Requirements
- Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care
- Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization
- Current CA Physical Therapist license
- Current Basic Life Support (BLS)
- Valid CA Driver's License with acceptable driving record
- All license and certifications must be current at time of hire and sustained throughout employment
- Must be able to visit patients in their homes and perform supervisory visits of field clinicians (12 patient visits each week)
- Ability to work hybrid schedule: 2 days in Santa Rosa office and 3 days in the field
