Care Manager for Erie Family Health Centers responsible for outreach and care coordination. Collaborating with health care teams to conduct assessments and manage patient needs.
Responsibilities
Conducts outreach to complete between 90-120 health risk assessments per month.
Conducts successful outreach to 120 disease management eligible or enrolled per month.
Collaborates with Erie’s Licensed Care Managers (LCMs), Transitions of Care (TOC) team, the Social Drivers of Health (SDOH), Health Promotions, and all other patient program team members to provide care coordination services and link with best fit program for patient needs.
Documents care team communication, resulting action steps, and activities in Erie’s electronic health record (Epic) for all participating patients, aligned with Erie and CARES Act standards (e.g., no medical jargon, etc.).
Documents all activities in additional external system as required/appropriate.
Requirements
High School diploma or equivalent required
Preferred: Community Health Worker training
Bachelor’s degree
At least 1 year of experience in community-based organizations, safety net settings, or with vulnerable population required
One year of customer service experience, preferably in a health care setting.
Experience identifying, referring to, and working with community-based organizations preferred.
Experience with health system navigation and patient education preferred.
Managed care, care management, or care coordination experience or exposure preferred.
Strong computer skills required: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).
Attentive to detail required
Strong interpersonal and oral communication skills required.
Comfort using phone-based translation services required.
