About the role

  • Behavioral Health Case Manager I overseeing telephonic case management for members with behavioral health needs. Conducting assessments and developing care plans to support client recovery and access to care.

Responsibilities

  • Perform case management telephonically within scope of licensure for members with behavioral health needs
  • Conduct assessments to identify individual needs and develop care plans
  • Monitor and evaluate effectiveness of care plans
  • Support member access to appropriate quality and cost-effective care
  • Coordinate with internal and external resources and collaborate with providers

Requirements

  • Requires MA/MS in social work, counseling, or related behavioral health field or degree in nursing
  • Minimum of 3 years clinical experience in social work counseling
  • Broad range of experience with complex psychiatric and substance abuse treatment
  • Current active unrestricted license (e.g., RN, LCSW, LMHC, LPC)
  • Experience in case management and telephonic and/or in person coaching preferred
