About the role

  • Community Care Manager leading day-to-day operations ensuring safe, high-quality care at TDHS Community Care services. Managing budgets, overseeing care coordination, and building a high-performing team.

Responsibilities

  • Lead the day-to-day operations of TDHS Community Care services
  • Ensure safe, high-quality, person-centred care meeting all funding, legislative and regulatory requirements
  • Coordinate care, oversee electronic health records and statutory reporting, manage budgets and routine expenditure
  • Ensure efficient service delivery in line with organisational policy
  • Build and support a high-performing team through workforce planning, recruitment, performance development and training

Requirements

  • Experienced leader in community or aged care
  • Strong knowledge of the Aged Care Act, Aged Care Quality Standards, CHSP, Home Care Packages and HACC
  • Proven experience in program management, budgeting, compliance, audit readiness and workforce leadership
  • Advanced communication and digital skills
