- Care Manager delivering patient-centered care in partnership with patients and providers. Collaborating with teams to improve health outcomes through disease management and prevention strategies.
Responsibilities
- Works in partnership with patients, families, caregivers, and providers to deliver targeted care
- Collaborates with interdisciplinary staff and stakeholders within the care continuum
- Promotes patients’ optimal health through disease prevention and management strategies
- Applies evidence-based interventions to populations for cost efficient and outcome-based care
- Aims for positive outcomes including disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates
Requirements
- Bachelor's Degree (Required)
- APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing
- BLS - Basic Life Support - American Heart Association
- RN - Registered Nurse - Ohio Board of Nursing
- 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources
- Computer skill and data entry
- Knowledge and skill in clinical assessment and disease management
- Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.
