About the role

  • Clinical Team Lead providing clinical oversight and support in residential settings for individuals with intellectual and developmental disabilities, ensuring high-quality care and staff supervision.

Responsibilities

  • Provide clinical oversight, staff supervision, and operational support across residential homes
  • Conduct initial nursing assessments and ongoing reassessments for clients
  • Review, monitor, and ensure accuracy of Medication Administration Records (MARs)
  • Manage medication reorders and coordinate with pharmacies
  • Monitor clients for changes in condition and report concerns appropriately
  • Accompany residents to medical appointments and communicate follow-up care plans
  • Supervise field staff across residential homes
  • Train, onboard, and mentor new hires
  • Conduct competency evaluations and skills validations
  • Perform routine site visits to ensure compliance with state regulations and agency policies
  • Provide coverage for call-offs and staffing shortages when needed

Requirements

  • Active LPN license in good standing
  • Minimum 2 years of nursing experience with IDD population and/or residential care settings
  • Strong knowledge of medication administration and documentation
  • Proven supervisory or leadership experience
  • Reliable transportation and ability to travel between multiple locations
  • Flexible availability, including evenings and occasional weekends
