About the role

• Consultant Pharmacist assisting in resolving pharmacy-related issues and coordinating services in long term care facilities. Collaborating with facility leaders to ensure compliance and quality care.

Responsibilities

• Assists the facility in identifying, evaluating, and addressing pharmacy-related concerns and issues that affect resident care, medical care, quality of life, and/or regulatory compliance.
• Responsible for collaboration with facility leaders and staff to coordinate pharmaceutical services within long term facilities.
• Recommends clinical reviews as required by federal and state regulation to company leadership.
• Advances issue resolutions and communicates early warning signs of potential issues.
• Represents the company when communicating with customers to promote and drive organic growth as well as new business.
• Conducts Executive Reviews and other customer meetings as required.
• Performs Medication Regimen Reviews (MRR) as outlined by regulation or contract for facility type.

Requirements

• Active pharmacist license in good standing in Massachusetts
• Immunization certified or willingness to become immunization certified
• Strong preference for pharmacist with additional license in New Hampshire and Vermont, or ability to obtain
• 2+ years of experience as a long term care pharmacist
• B.S. in Pharmacy or PharmD degree required
• Certification in geriatric pharmacy preferred.
