About the role

  • Bereavement Counselor providing individual and group counseling for Hospice patients and grieving families. Engaging in documentation and client contact to support timely care and compliance.

Responsibilities

  • Provide individual and group counseling and therapy for Hospice patients, family members and community individuals who have experienced the loss of a significant person through grief.
  • Demonstrate contact of all clients within established time frames and provide appropriate documentation.
  • Document at least two additional attempts to reach an assigned client before closing the case.
  • Demonstrate a willingness and ability to manage tasks and case loads assigned, from any departmental program, in an effective, productive and timely manner, meeting the standards that reflect expected productivity.
  • Demonstrate a willingness to provide counseling services such as on call crisis intervention, individual and group, on a flexible schedule as needed, including some week nights, weekends and at camp.
  • Document all client contacts and care plans accurately according to department and organization policy in a timely manner.

Requirements

  • master's degree in counseling, social work, marriage and family therapy, or related field (if hired after 5/26/2024)
  • bachelor's degree in counseling, social work, marriage and family therapy, or related field (if hired prior to 5/26/2024)
  • Clear Level 2 background check
  • Must possess a valid Drivers' License
  • Current auto insurance
  • Reliable transportation
  • Professional Licensure as required by state where care is being provided.
