About the role
- Bereavement Counselor providing individual and group counseling for Hospice patients and grieving families. Engaging in documentation and client contact to support timely care and compliance.
Responsibilities
- Provide individual and group counseling and therapy for Hospice patients, family members and community individuals who have experienced the loss of a significant person through grief.
- Demonstrate contact of all clients within established time frames and provide appropriate documentation.
- Document at least two additional attempts to reach an assigned client before closing the case.
- Demonstrate a willingness and ability to manage tasks and case loads assigned, from any departmental program, in an effective, productive and timely manner, meeting the standards that reflect expected productivity.
- Demonstrate a willingness to provide counseling services such as on call crisis intervention, individual and group, on a flexible schedule as needed, including some week nights, weekends and at camp.
- Document all client contacts and care plans accurately according to department and organization policy in a timely manner.
Requirements
- master's degree in counseling, social work, marriage and family therapy, or related field (if hired after 5/26/2024)
- bachelor's degree in counseling, social work, marriage and family therapy, or related field (if hired prior to 5/26/2024)
- Clear Level 2 background check
- Must possess a valid Drivers' License
- Current auto insurance
- Reliable transportation
- Professional Licensure as required by state where care is being provided.
