- Therapist providing therapeutic interventions and assessments for clients with behavioral health diagnoses at Centerstone. Aiming to promote personal growth and resiliency through individual and group therapy.
Responsibilities
- Provides therapeutic interventions to promote personal growth and resiliency
- Performs continuing assessment of the client’s needs and functioning
- Facilitates therapy sessions for a client or group of clients
- Assists client in developing an individualized service plan
- Monitors client progress in treatment
- Updates service plans as appropriate
- Conducts intake for client entering services
- Conducts assessment and write assessment summaries
- Assists clients with making linkage with internal and external resources
- Maintains appropriate contact with referral sources
- Assists client in making discharge plans and provide follow up services
Requirements
- Master’s Degree in a human services related field
- Experience providing mental health services preferred.
