Registered Nurse providing in-home hospice care in San Antonio for Pacific Hospice. Collaborating with interdisciplinary teams to enhance patients' quality of life during end-of-life journeys.
Responsibilities
Provide comprehensive and compassionate nursing care to patients in their homes
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct thorough assessments of patients' physical and psychological status
Administer medications according to physician orders
Provide education and support to patients and their families
Offer emotional support and counseling to patients and families
Collaborate with the interdisciplinary team for continuity of care
Maintain accurate and up-to-date patient records
Provide bereavement support to families and caregivers
Stay informed about current trends and best practices in hospice and palliative care
Requirements
Current and unrestricted RN license in the State of Texas
Minimum of 2 years of proven clinical experience in hospice, palliative care
Bilingual English/Spanish preferred
Proficient with computers: Axxess EHR or other hospice EHR; email; able to use Slack and other platforms
Strong case management skills, including assessment, care planning, coordination, and documentation
Ability to provide compassionate and empathetic care to patients and families
Excellent verbal and written communication skills
Demonstrated ability to work reliably and collaboratively in an interdisciplinary team
Familiarity with hospice regulations, including Medicare Conditions of Participation and Joint Commission standards
