About the role

  • Provide access to appropriate healthcare and community services
  • Assist with outreach, engagement, and continuous support for a cohort of members
  • Develop and coordinate community resources to address medical, behavioral, violence prevention, and social service needs
  • Assess risk, perform crisis interventions, and ensure safety through tailored safety plans
  • Collaborate with members to assess care options and develop and update service plans
  • Educate members about mental and physical health, abuse prevention, medication, and available resources
  • Maintain accurate and up-to-date records of all case management activities

Requirements

  • Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution.
  • At least 2 years of experience in a related role, preferably working with at-risk adolescents and young adults.
  • Crisis intervention training or similar certification is required.
  • Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, suicide ideation and social issues.
