About the role
- Care Manager providing support to individuals and families in Louisiana's Teen Suicide Prevention Program. Engaging members, coordinating care, and enhancing well-being in a collaborative environment.
Responsibilities
- Provide access to appropriate healthcare and community services
- Assist with outreach, engagement, and continuous support for a cohort of members
- Develop and coordinate community resources to address medical, behavioral, violence prevention, and social service needs
- Assess risk, perform crisis interventions, and ensure safety through tailored safety plans
- Collaborate with members to assess care options and develop and update service plans
- Educate members about mental and physical health, abuse prevention, medication, and available resources
- Maintain accurate and up-to-date records of all case management activities
Requirements
- Bachelor’s degree in psychology, social work, or another applicable behavioral health discipline from an accredited institution.
- At least 2 years of experience in a related role, preferably working with at-risk adolescents and young adults.
- Crisis intervention training or similar certification is required.
- Knowledge of local resources, social determinants of health, mental health, substance abuse disorders, interpersonal violence, suicide ideation and social issues.
