About the role

  • Case Manager position at VOA assisting seniors in managing homelessness and transitioning to independent living. Focus on developing self-reliance and connecting clients with necessary services.

Responsibilities

  • Develop case management plans to assist participants in developing a greater degree of self-reliance and independence
  • Connect with clients and facilitate them successfully connecting with other services
  • Maintain a strong and collaborative relationship with landlords and program partners
  • Assist clients with purchasing basic necessities
  • Counsel clients on how to remain stable in housing long term (through budgeting, paying bills, housekeeping, community building, transportation awareness, increasing income, healthy socialization, and maintaining a connection to services such as mental health, medical, AOD, etc.)
  • Complete appropriate needs assessments for all clients
  • Determine client eligibility for additional services
  • Participate in outreach to locate new clients
  • Lead client-based groups and meetings, including individual one-on-one appointments
  • Explain and link clients to agency and community services
  • Assist clients in accessing community services when appropriate
  • Provide crisis intervention
  • Inform clients of housing options
  • Maintain a caseload of clients
  • Ensure all intake paperwork is complete and accurate
  • Request appropriate releases of information as required
  • Enter client data into HMIS and Onesite accurately and within 24 hours
  • Assist in transitioning clients into permanent housing
  • Complete the individualized service plan with client-driven goals
  • Review individualized service plan with client and update as progress is made
  • Advocate on behalf of the client
  • Attend other agency events and meetings to collaborate on our services
  • Respond to agency inquiries and return calls to other agencies within one working day
  • Maintain client confidentiality
  • Participate in Volunteers of America and outside training sessions
  • Perform other reasonably related duties as assigned by the supervisor

Requirements

  • Bachelor’s or associate degree in social work, human services, or a related field preferred
  • Basic knowledge of property management, fair housing, and renter responsibilities
  • Ability to communicate utilizing Motivational Interviewing and Trauma Informed Care techniques required
  • Demonstrated ability to communicate and comprehend oral and written instructions
  • General knowledge of issues and challenges unique to low-income, homeless, minorities, and persons with disabilities
  • Ability to utilize a range of interventions to work with clients with varying needs and levels of functioning
  • CPR and First Aid training within 90 days of employment date (recertification as necessary)
  • Valid California driver’s license and ability to meet the organization’s insurance carrier guidelines
  • Undergo a criminal background check
