About the role
- Nurse overseeing health program visits and patient monitoring within Unimed Porto Alegre. Responsibilities include patient care, auditing, and quality compliance in health services.
Responsibilities
- Conduct virtual and home visits, or visits at Unimed facilities, and monitor these patients;
- Monitor the care process;
- Promote routines aligned with the culture of quality and patient safety;
- Audit and evaluate processes with a focus on continuous improvement;
- Monitor patients with chronic diseases and/or other pathologies according to the risk factors of the care pathways, via telephone contact;
- Document interventions / patient progress in designated systems/platforms;
- Monitor implementation of action plans and care plans for projects approved by ANS, in accordance with the care pathway;
- Generate and analyze program indicators, proposing improvements and action plans;
- Manage individual care plans for patients participating in the care pathways;
- Monitor / evaluate activities carried out by third-party providers for the cooperative's clients.
Requirements
- Bachelor’s degree in Nursing.
- Experience in oncology;
- Experience in palliative care.
