About the role

  • Youth Case Manager in Oasis Link-In-Youth team, providing support to young people and families. Focused on evidence-based case management and community engagement in Australia.

Responsibilities

  • Deliver holistic, evidence-based case management to young people and families, supporting at least 25 clients annually
  • Build family capacity through targeted support and engagement activities
  • Facilitate community connection by organising activities that promote inclusion and participation
  • Provide advocacy and practical support, including pathways to employment, education, counselling, mentoring, and material aid
  • Coordinate referrals and assist clients in navigating services such as community networks, allied health, and employment supports
  • Deliver flexible, client-centred support across home, community, online, and site-based settings
  • Strengthen community capability through education, skills development, and volunteer engagement
  • Conduct intake, assessments, risk evaluations, and develop SMART case plans using trauma-informed approaches
  • Maintain accurate, timely documentation and complete regular case reviews
  • Collaborate with stakeholders to deliver coordinated, strengths-based, child-centred and family-inclusive care
  • Contribute to program development, continuous improvement, and team effectiveness
  • Uphold child safety, mandatory reporting, and safeguarding requirements
  • Build strong partnerships and contribute positively to team culture and shared outcomes
  • Deliver culturally responsive and inclusive services to diverse communities

Requirements

  • Tertiary qualification in social work welfare, or related field (degree preferred)
  • Current NSW Working with Children Check
  • Current NSW Driver’s Licence
  • Eligibility for registration with peak body (where applicable)
