About the role

  • PRN Physical Therapist providing therapy services for seniors in home settings across Ohio. Collaborating with healthcare teams to restore function and improve mobility for residents.

Responsibilities

  • Evaluate and develop individualized treatment plans to restore function, improve mobility, and relieve pain
  • Provide direct therapy services in the home setting, helping residents achieve their highest level of independence
  • Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care
  • Document patient progress and outcomes accurately and timely
  • Educate patients, families, and caregivers on therapy techniques and home exercise programs

Requirements

  • Current state licensure as a Physical Therapist (PT)
  • Graduate of an accredited Physical Therapy program
  • Home health experience preferred, but not required
  • Compassion, adaptability, and a passion for serving seniors
