About the role

  • Physical Therapist providing outpatient rehabilitation services in Illinois. Evaluating patients and implementing individualized treatment plans based on specific healthcare needs and functional goals.

Responsibilities

  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
  • Establish and implement individual treatment plans based on evaluation results and goals
  • Monitor and evaluate outcomes, adjusting treatment plans as needed
  • Communicate and collaborate with stakeholders to assist in achieving optimum patient care
  • Complete all required documentation following department policy

Requirements

  • Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation
  • Licensed Physical Therapist in the State upon which you work
  • Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) obtained within 90 days unless exempt
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served
