About the role

Responsibilities
- Interprets physician referrals and completes evaluations utilizing appropriate methods
- Establishes a plan of care and implements individual treatment based on evaluation results
- Monitors and evaluates treatment outcomes; adjusts treatment plans as needed
- Plans for and completes patient discharge including providing written home programs and patient/family teaching
- Responsible for cleaning up after patient treatment and keeping area neat
- Demonstrates skill in use of modalities and exercise equipment
- Participates in performance improvement activities related to patient care outcomes
- Pursues professional growth through continuing education courses and department inservices
Requirements
- Bachelor's Degree in Physical Therapy
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months
- Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois)
- Intermediate computer skills
- Excellent communication and interpersonal skills
- Demonstrated organization, decision-making, and problem solving skills
