Occupational Therapist role in home health providing therapy services to patients in their homes. Key tasks include assessments, treatment planning, and collaboration with healthcare teams.
Responsibilities
Work 100% FTE, Day shift, Monday through Friday, Variable hours, generally 0800 - 1630, No weekends or holidays.
You will work at 2550 Charles St.
You may be eligible for a $15,000 sign on bonus.
Requirements
Graduate of an accredited Occupational Therapy program. Required
Current license from the State of Illinois or in the process of acquiring licensure. Required
Valid State driver’s license and proof of liability insurance. Required
CPR certification/bi-annual. Required
An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
