About the role

  • Registered Nurse providing compassionate in-home hospice care in San Antonio. Collaborating with interdisciplinary teams to enhance patients' end-of-life quality of life.

Responsibilities

  • Provide comprehensive and compassionate nursing care to patients in their homes, ensuring their physical, emotional, and spiritual needs are met.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and their families, considering their unique needs, goals, and preferences.
  • Conduct thorough assessments of patients' physical and psychological status, including pain management, symptom control, and psychosocial support, ensuring ongoing evaluation of their condition.
  • Administer medications, including pain management and symptom control medications, according to physician orders, and educate patients and families on medication management and potential side effects.
  • Provide education and support to patients and their families regarding the disease process, treatment options, pain management techniques, and end-of-life care, promoting understanding and involvement in the care plan.
  • Offer emotional support and counseling to patients and families, addressing their fears, concerns, and coping mechanisms during the end-of-life journey.
  • Collaborate with the interdisciplinary team, including physicians, social workers, spiritual care providers, and volunteers, to ensure continuity of care, effective communication, and holistic support.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements, hospice policies, and best practices.
  • Provide bereavement support to families and caregivers following the loss of a loved one, offering resources, counseling, and referrals as needed.
  • Stay informed about current trends, best practices, and advancements in hospice and palliative care through participation in continuing education opportunities and professional development activities.

Requirements

  • Registered Nurse License: Current and unrestricted RN license in the State of Texas
  • Minimum of 2 years of proven clinical experience in hospice, palliative care, demonstrating expertise in end-of-life care, pain management, and symptom control
  • Bilingual English/Spanish preferred
  • Proficient with computers: Axxess EHR or other hospice EHR; email; able to use Slack and other platforms
  • Case Management Skills: Strong case management skills, including assessment, care planning, coordination, and documentation
  • Compassion and Empathy: Ability to provide compassionate and empathetic care to patients and families in a professional manner during the end-of-life journey, demonstrating sensitivity to their physical, emotional, and spiritual needs
  • Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and interdisciplinary team members
  • Professional Team Player: Demonstrated ability to work reliably and collaboratively in an interdisciplinary team, fostering a professional, positive, supportive work environment
  • Regulatory Knowledge: Familiarity with hospice regulations, including Medicare Conditions of Participation and Joint Commission standards
  • Reliable transportation.
