About the role
- Home Health Physical Therapist developing customized care plans for patients. Collaborating with healthcare professionals for improving patients' mobility and overall physical function.
Responsibilities
- Evaluate and assess patients' physical conditions to develop tailored treatment plans
- Implement and adjust physical therapy interventions focused on mobility, strength, balance, and function
- Educate patients and caregivers on therapeutic exercises, techniques, and safety precautions
- Collaborate with physicians and other healthcare professionals to coordinate patient care
- Maintain accurate, timely documentation of patient progress and treatment plans
- Participate in on-call rotation (if required)
Requirements
- Licensed Physical Therapist (PT) in [state]
- Minimum of 1 year of clinical experience (home health preferred but not required)
- Excellent communication, problem-solving, and organizational skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and valid driver’s license
- Knowledge of Medicare/Medicaid guidelines is a plus
