About the role

  • Occupational Therapist providing personalized in-home therapy and support for Western Montana's leading home health agency. Flexible scheduling with generous benefits and a supportive team.

Responsibilities

  • Conduct comprehensive evaluations and develop individualized treatment plans for home-based patients, coordinating with physicians and the care team as needed.
  • Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve.
  • Complete and score OASIS assessments accurately, collaborating with the Quality team to ensure compliance.
  • Maintain knowledge of payer guidelines and Medicare Conditions of Participation.
  • Participate in interdisciplinary case conferences and Quality Assurance & Performance Improvement (QAPI) initiatives.
  • Supervise Occupational Therapy Assistants (OTAs) and provide oversight to CNAs when OT is the sole active discipline.
  • Manage patient care throughout the episode, including admissions, 30-day functional assessments, and discharges.
  • Document services and communications promptly and accurately using point-of-care tools.
  • Coordinate daily schedules efficiently to maintain continuity of care and minimize travel time.
  • Maintain current Montana OT licensure and complete all continuing education requirements.
  • Adhere to ethical standards and agency policies while representing PIHC’s mission and values.
  • Perform additional duties as assigned by the supervisor.

Requirements

  • Bachelor’s degree or higher from an accredited Occupational Therapy program
  • Current licensure to practice Occupational Therapy in the State of Montana
  • Valid driver’s license and reliable transportation with appropriate auto insurance
  • Two years of occupational therapy experience preferred (home health experience a plus)
