About the role

  • RN Case Manager facilitating transitions for patients from LTAC to lower levels of care. Engaging with interdisciplinary teams to ensure continuity and compliance throughout the process.

Responsibilities

  • Facilitate safe, timely, and well-coordinated transitions of patients
  • Attend scheduled onsite days at assigned LTAC facilities
  • Participate in interdisciplinary rounds and discharge planning meetings
  • Review provider discharge readiness decisions and ensure orders are initiated promptly
  • Identify barriers to discharge and escalate as necessary
  • Support family and caregiver education on post-discharge instructions
  • Complete discharge documentation, coordination notes
  • Arrange logistics including transportation, DME, pharmacy coordination
  • Confirm successful transfers and monitor members for 30-day readmission

Requirements

  • Registered Nurse (RN) or equivalent clinical training preferred
  • Associate degree in Nursing, Health Sciences, Social Services, or related field minimum
  • Minimum 3 years’ experience in care coordination, discharge planning, or case management within LTAC, acute hospital, SNF, or managed-care environment
  • Familiarity with CalAIM, ECM, or Community Supports preferred
  • Strong interpersonal skills
